Registering for the first time from abroad

Please note:

• Each person/family member should complete a separate form for themselves. Parents can complete a separate form on behalf of their minor child or children
• A Visa is not required for nationals of an EU Member State / EEA Member Country / Switzerland
• The form does not work correctly when using Google translate. Please switch off Google translate before using this form

Required documents:

• A valid passport (front cover + personal details + signature), or identity card (copy of both sides)
• Birth Certificate / Marriage Certificate / Divorce Certificate (if this took place outside the Netherlands)*
• A signed lease or proof of ownership document
  Or a signed declaration of the property's main occupant (to proof you are a resident). Download the declaration form here. Please note that in case of live-in, copy passport or ID of the consenting party is also required.

If you're a non-EU resident, we also need:

• Residence permit / Visa (not a tourist visa) or a residence endorsement sticker. (copy of the corresponding visa page in your passport)

*) You can hand these documents in any time after your registration.
